 Case Management Services

Case Management services are available for people living with HIV or AIDS in our coverage area. Referrals are provided for clients outside of our service area.

Health Assistance

Case Managers help clients learn about their unique health needs and the programs available to help them maintain their health.

Services include:

  • Health Education
    Focusing on the importance of continued treatment, risk reduction for partner(s), education on health care coverage options, understanding and reducing risks during pregnancy, and utilizing safer sex practices.

  • Health Support
    Our case management department offers individualized, client-centered HIV support in a small private setting, providing clients with prompt access to staff and short wait times. Clients can receive HIV navigation help and assistance with medication adherence. Offices can also assist with lab work including CD4 and viral load counts, and HIV medication facilitation.

Nutrition Services

Clients have access to nutritious food items and nutrition counseling in order to promote overall health and well-being. These services include access to:

  • Grocery/prepared meal delivery

  • In-office food bank or food baskets

  • Personal hygiene products

  • Household cleaning supplies

  • Referrals to community nutrition programs

Financial Assistance

Services include:

  • Health Insurance Premium and Cost-Sharing Assistance
    For those with financial need, case management may be able to help with health and/or dental premiums and cost-sharing with medical co-pays and/or deductible bills.

  • Rental Assistance

  • Utility Assistance

  • Replacement State IDs/Driver's Licenses

Mental Health

Clients have access to mental health services provided by our Masters Level Clinician, Priscilla Morataya.

  • Cognitive Behavioral therapy with a client-centered approach

  • Types of Sessions available:

    • ​Individual

      • Short-term​

      • Long-term

    • Family or couples' sessions​​

  • Support coping with various issues, including:

    • Mood and anxiety disorders

    • Healing from traumatic experiences

    • Creating healthy relationships

    • Setting boundaries

Transportation Assistance

Transportation is available to clients who need non-emergency access to medical care and/or service appointments.

  • Mileage Reimbursement
    Available for clients who use their own vehicle to attend medical appointments.

  • Direct Transportation
    Case Managers may be able to directly provide transportation for clients, as permitted by policy.

  • Public Transportation
    Access to tokens, bus passes, etc. for clients.

  • Taxi/Ride-Sharing
    If available and no other forms of transportation are accessible, case managers can assist clients in getting taxis or ride-shares (Uber, Lyft, etc.) to their next medical appointment.

Housing Assistance

Individuals who are homeless or at risk or of becoming homeless may be eligible for housing education and support services, including:

  • Emergency housing assistance

  • Transitional housing

  • Help with 1st month's rent and security deposit

  • Short-term rental assistance

  • Long-term rental assistance

Note: Moving expenses are not available

Note: In the Northcentral region, Caring Communities directly handles clients' housing needs. In the Northeast region, we refer to our Northeast regional funder, United Way of the Wyoming Valley, who provides housing.

Case Management offers many additional services for clients including a mail-order pharmacy program, help accessing oral health care, HIPAA-compliant telehealth technology, mental health and wellness support, and social events.

Support Group Sessions for Case Managed Clients

Clients of Case Management are invited to attend support group sessions covering various topics, with open-discussion time as well.

Sessions will be held simultaneously at our Wilkes-Barre and Bloomsburg offices. Clients may join via phone, telehealth equipment, or in-person. A support group facilitator will be available at both locations to provide additional support.
